Family Case Manager

• Actively coordinate assistance provided to children and families through the child protective services case process using the Family Teaming process of a strength-based family-centered approach including: child and family assessments; development and management of service and case plans; use of resources including formal and informal supports and services to ensure child safety; evaluation and monitoring of case progress and movement toward case closure. • Responsible for decision-making, recommendations, and case activity that supports appropriate use of out-of-home placements, children's involvement, recommendations for permanency including reunification, transfer of guardianship, adoption, etc.• Conduct home visits, hospital and school visits, and attends court hearings.• Participate and testify in court hearings while collaborating with legal parties following court orders for each individual case.• Perform adoption case management/finalization including negotiating adoption assistance subsidies and completing court work. • Adhere to strict timelines, best practice models, and state policies/procedures regarding documentation and service provision.• Participate in work groups including; Future Leaders Group, State/Federal Policies and Procedures, and Family Connectedness.• Workgroup responsibilities include development and implementation of new services, front line staff hiring, correct usage of federal/state/local/agency policies and procedures, and one on one mentoring to support recently hired staff.
