Oversaw the Quality of Care and population health management of 1778 participants who are nursing home eligible. Helped to improve the medical utilization of services and ensured revenue was captured through appropriate education and compliance programs. Particular initiatives I led include- Ensured appropriate documentation of diagnoses reflecting the frailty of the population leading to improved revenue accuracy and Medicare reimbursement - Leading process improvements and solutions resulting in improved utilization, quality and patient satisfaction by decreasing emergency room utilization, decreasing the need for long term care through various housing options allowing people to age at home etc. -Decreasing LOS in SNFs while keeping the SNF admissions steady-Decrease Long Term Care Admissions by providing community supportive housing options and flexing the PACE model-Decrease ER/Observation stays significantly through outreach and education-Recruitment of Medical Staff and Nurse practitioners with development of a retention program for providers in 2019

I serve as Medical Director of Evidence based Programs at Hebrew SeniorLife and Medical Director of the Healthy Living Center of Excellence (HLCE), an organization funded by the John A. Hartford Foundation and the Tufts Health Plan Foundation. The HLCE represents a unique collaboration of community-based organizations, aging service providers, health care systems, governmental agencies, and healthcare payors; all with the shared goal of transforming the traditional health care delivery system. It represents an integrated delivery system which leverages the expertise and resources of the community to achieve better care, better health and lower costs. The Center strives to promote the independence of older adults by empowering them and their families to take more active roles in their care by offering quality, proven interventions addressing issues including, but not limited to management of chronic illness, methods to prevent and manage fall and promote physical activity and proper nutition. The Center’s mission and vision are achieved through the implementation of evidence- based disease management programs to empower older adults to actively manage their conditions. These programs all occur in community settings and are designed to empower adults living with one or more chronic conditions to take a more active role in their health care. Programs, including the Stanford University Chronic Disease Self-Management Programs (called “My Life, My Health” in Massachusetts), focus on multiple behavior changes to achieve better health.

This program is being used to develop personalized wellness and life care plans for older adults that will be utilized across the health care continuum. Serves as one of the principals in the development of this programs and is actively engaged in helping communities implement it. Vitalize 360 is a coaching and assessment process for older adults that uses art and science to promote optimal wellness for successful aging. The program combines an award-winning, innovative, person-directed approach to wellness coaching with the power of information derived from a scientifically-grounded assessment system developed by InterRAI. This programs is now being utilized in 33 CCRCs, housing sites and CCRC at home programs around the country and is continuing to grow throughout the country.

Created the strategic direction of Hebrew SeniorLife (HSL) Medical Group in home and community based services, long-term care and post acute care which still exists to this day. I was a faculty member of the Institute for Aging Research at Hebrew SeniorLife and I am working to connect research to health care systems clinical venues. I still serve on the faculty at Harvard Medical School and helped implement the approach and the attainment of the Harvard Medical School affiliation that occurred during my tenure.

Led development and implementation of a geriatric consultation service in a Patient Centered Medical Home for an Accountable Care Organization (Dedham Medical Associates) with 30 internist to develop a systems approach to managing the high risk elder population. In particular, the practice focused on patients with cognitive impairment and dementia, palliative care and multiple morbidities. This program has been described as the "Gold Standard" for Frail Elder Care in Massachusetts according to the medical leadership of the Tuft's Medicare Advantage Program at that time. Documented cost savings of $2500 per patient in 6 months time post comanagement using evidence based approaches and engagement was documented and validated.

Served as the Department Chairman at Lahey Clinic in Burlington, Massachusetts where I developed innovative geriatric evaluation and management units in skilled nursing facilities that resulted in improved population health outcomes at lower costs. In addition, I led the development and implementation of a care management program that was highly successful in managing the most complex older adults in a Medicare HMO program Recognized two years in a row as top 50 Geriatric Departments by US News and World Reports
