About Robert Schreiber

My focus is on creating and developing a Social Movement to allow older adults to live their best possible life. Building a community system of care for marginalized and racially diverse complex and frail elders, allowing them to have purpose and meaning in their lives in a manner that allows inclusion and is financially sustainable for society and their families. As part of this work we published a "Roadmap for Care of Older Adults" in a white paper entitled PCMH and Care of Older Adults through the John A. Hartford Foundation.This learning has been applied to PACE populations so that we can scale this model to vulnerable. frail populations that are nursing home eligible living in the community. There is no standardized model for doing this and our goal is to create one using PACE principles is the goal standard. The Covid Pandemic has created a sense of urgency to scale community models of care for vulnerable older populations rather than depending on nursing homes for ones final years. Baby boomers will not accept nursing homes as an option so the need is here now. There is a national initiative through the Billions Institute and the National Pace Association to scale this model of care from 40, 000 people to 200,000 people in five years and over 1 million people int 10 years. The clock is ticking and the hard work is just beginning.

The Center’s mission and vision are achieved through the implementation of evidence- based disease management programs to empower older adults to actively manage their conditions. These programs all occur in community settings and are designed to empower adults living with one or more chronic conditions to take a more active role in their health care. Programs, including the Stanford University Chronic Disease Self-Management Programs (called “My Life, My Health” in Massachusetts), focus on multiple behavior changes to achieve better health.
