I'm very fortunate to be part of the incredible team that makes up the AMA's Accelerating Change Transformation Team (ACTT)!ACTT’s mission is to enable system transformation in four ways:Content: Find, curate and build materials that help members and partners understand and implement the transformative changes.Capacity: Help members and partners build the skill sets they need to implement the transformative changes.Support: Help partners (especially PCNs) in their organizations to do the hard work of leading and implementing change.Influence: Influence policy, practice, funding and services of other groups (founders, AHS, HQCA, etc.) to maximize the chances of successful transformative change.ACTT's top transformation priorities revolve around advancing Patient's Medical Home and Health Neighborhoods. Specific projects include CII/CPAR, improving Specialty Access (Alberta Surgical Initiative, FAST, etc.) and transitions in healthcare (H2H2H), supporting physician leaders, practice facilitators, and other members of the Health Transformation Workforce, and creating and supporting communities of practice (EMR Networks, PF network, etc.).

I joined the Bridgepoint FHT in the middle of the COVID-19 pandemic, and was continuously impressed by this team of dedicated professionals. Alongside the Medical Director, I facilitated our multidisciplinary team in their delivery of accessible, high-quality, primary care throughout the life cycle.

As acting program lead (just over 1 year), I managed the diabetes education program staff and operations, collected and analyze workload and quality statistics, generated quarterly reports and annual work plans, handled patient relations, lead our team in program planning activities, maintained medical directives, assisted with hiring, training and developing new talent, and ensured that the diabetes program aligned with the broader organization's strategic goals. I was also an active Quality Improvement Committee member, former JHSC member, and a key EMR expert for the entire FHT.In my clinical role, I provided comprehensive individual and group counselling for people living with Type 1, Type 2, Gestational and Prediabetes. I also assisted clients with nutrition therapy for a variety of concomitant or comorbid conditions, including hypertension, dyslipidemia, chronic kidney disease, hepatitis, gastrointestinal disorders, bariatric surgery, allergies and intolerances, disordered eating, etc. I also served as preceptor for many RD-candidate graduate students.

I provided individual and group counseling for students for a wide variety of dietary issues, including but not limited to eating disorders, gastrointestinal/digestive problems, metabolic disorders, allergies/intolerances, vegetarianism, anemia, nutrient deficiencies, prenatal nutrition, etc.Charting at this position was via Jonoke and Accuro, and I designed custom templates for both systems.

At this CHC servicing predominantly First Nations clients, I was a key part of the team that created a new diabetes education program in a new location. Clinical duties included individual and group counselling for Type 2, prediabetes and at-risk clients at the CHC or in remote community settings. This position required extensive interdisciplinary team work, program planning, and community engagement to meet the CHC's mandate to provide culturally sensitive care. I was fortunate to be involved in the planning of several community events, including National Aboriginal Day, the Great Canadian Kayak Challenge, National Aboriginal Diabetes Awareness Day, and the Timmins Diabetes Expo.I was also the diabetes program's website designer and administrator, and EMR “super-user” (Nightingale); responsible for creating custom electronic provider- and clinic-specific charting templates and care flow sheets, extract statistics, troubleshooting user issues, and training new employees.
