I’m privileged to lead a group of integrated social service programs supporting patients at-risk for high utilization and poor health outcomes due to comorbid medical and social complexity. Using a community health approach, we partner with hospitals, clinics, and communities to design health services that promote greater access, coordination, and equity of care for vulnerable patients and families. Key innovations include intensive care coordination for emergency department high utilizers, financial social work, behavioral health counseling for cancer patients, home visitation for at-risk mothers and infants, and primary care social work. We also operate a postgraduate marriage and family therapy training program accredited by Commission on Accreditation for Marriage and Family Therapy. Leadership expertise includes: collaboration science, system innovation and design, strategic planning, project management, driving results through data collection and analysis, talent development and coaching, and solution-focused patient care navigation.
