• Provide support to three program treatment teams of the Wraparound program.• Provide case management services to Wraparound clients and their families in home, school, juvenile placements, and in the community. • Supervise and assist program staff with day to day issues and crisis situations. • Ensures all direct reports are performing effectively and efficiently by consistently meeting contract and/or agency deadlines. o Created and reconstructed internal tracking system that identifies the status of each client progression from intake to disenrollment. o Created clear and concise date system that provided service delivery expectations, case by case safety crisis plans, outcome measures, and court mandates. • Coordinate and facilitate Wraparound program in various Service Areas with intra and inter agency team members. o Conduct monthly meetings with DMH & Probation Wraparound liaisons to ensure clients are provided proper mental health care. o Enroll, complete assessments, client treatment plans and complete applicable DMH documentation to all wraparound clients in Service Area 2, 4 and 6.

• Provide non-traditional counseling, therapy, and case management services to ensure successful transition of clients and their families to community-based resources.• Facilitate treatment team meetings, which can include youth, family members, community members, Probation Officers and or Child Welfare Workers.• Collaborate closely with physiatrists, treatment team, family members, school, and community personnel to appropriately coordinate client care services. o Quick response to crisis situations (severe mental health and behavioral issues). o Consult with psychiatrists on client medication changes, issues, and compliance. o Develop comprehensive treatment plans to manage and respond appropriately to problematic behaviors.o Assist with ensuring parents and teachers implement consistent behavior therapy techniques at home and at school. o Participate in Individualized Education Plan meetings.• Complete and maintain accurate clinical treatment plans, progress notes, and discharge plan documentation.• Presents youth treatment progress to community partners (DMH, DCFS, DPO’s).• Provide interventions to clients such as anger management techniques, relaxation skills, impulse control, social and emotional coping skills, and functional life skills.

• Productively assisted caregivers in coping with challenging client behaviors through behavioral interventions.• Provided one on one adjunctive behavioral mental health services. • Supported caregivers to implement clear expectations and consequences with clients.• Collaborated with parents, teachers, and mental health professionals to maintain continuity of structured treatment plans. • Encouraged families to use differential reinforcements and behavioral modification interventions to reduce targeted maladaptive behaviors.
