- Assesses, plans, implements, and evaluates options and services required to meet an individual's discharge planning and other psychosocial needs- Work primarily in the ICU, surgical unit and ER department assigned to complex cases- Possess a working understanding of ICU medical terminology and processes- Resolve high-risk situations by coordinating with patients, their families, the medical team, and other resources to ensure that patients are discharged to a safe environment
