In this role I act as a discharge planner, working with multiple stakeholders across Kaiser and other organizations to ensure that patients are discharged to a safe environment that is appropriate for their physical condition. I interfaces directly with patients and families, and advise multiple departments within the hospital – including Emergency, Inpatient and Post-Surgical care departments – regarding safe discharge scenarios for patients.
