Directs a team of clinicians who are:• Accountable for deployment of industry leading, innovative care management solutions to customers supported by our account teams in the Corporate/Independent and Hospital IDN space. • Accountable for the delivery of expert knowledge of Care Management resources, evolving care management industry trends and delivery/implementation of care management initiatives for assigned Corporate Independent and IDN/Hospital customers with a “whole patient” approach. Explained difficult clinical, patient outcome and adherence concepts to HCP and staff in the areas of Lupus, Breast Cancer, Diabetes, Asthma, COPD and Heart Failure.• Identify customer care management initiatives that can be developed and extended across customer segments including Payer, Hospital, and Provider Groups to enhance patient outcomes. I work with the hospitals and IND's throughout the Pacific Northwest to help embed GSK resources for patients and health care providers that will assist in improving patient self-management of their chronic diseases.

Discussed gaps in care and resources with health care providers in the outpatient setting to establish ways GSK resources could improve care to their patients with Asthma and COPD. I provided staff and provider trainings on how to better engage a patient in a discussion on self-management. I also provided educational presentations to staff and providers on disease state and updates on guidelines via our GSK approved resources.
