I am a health and human services professional with over 4 years of direct client services with both aging and youth populations. For 2 years, I worked as a home health aide promoting the health and well-being of my clients by supporting activities of daily living, scheduling appointments, providing medical reminders, and following daily care plans. During this time, I also returned to school to earn my CNA certification gaining additional clinical knowledge and experience. For 2 years, I worked as a Youth Counselor at a homeless shelter for teen parents and their babies. In that role, I worked with case managers and youth to develop case plans, referred youth to community resources, monitored the physical and behavioral health of clients, and facilitated program intakes and exits to ensure successful transitions. In my current role, I work for a virtual behavioral health clinic, answering phone calls and making outbound calls. I work closely with hospital emergency departments to coordinate providing level of care assessments. For patients that require a higher level of care, I facilitate bed/provider searches and ambulance transportation. I also keep our resource list current.
