Successfully managed another student with diabetes through careful monitoring of blood sugars throughout the school day.

Successfully opened new health center providing primary care services to the uninsured and underinsured residents in Lake County. New patient and Visit numbers increased annually.Developed new community outreach programs for patients on nutrition, pre-diabetes and diabetes, benefits of regular exercise, dental health and woman’s health care, which focused on the importance of leading a healthy lifestyle. Patients participating in these programs achieved weight loss goals and improved chronic disease management.Developed the clinic workflow and implemented the Diabetes Prevention and Treatment Program which provided a fully comprehensive and multidisciplinary visit for our diabetic patients. Patients attained better control of their diabetes and achieved goals to eat a healthier diet and exercise regularly.Identified increased need to provide prenatal and women’s health services to an underserved and uninsured patient population. Collaborated with the Family Medicine Residency Program at Glenbrook Hospital and Highland Park Hospital Labor and Delivery to provide these services. Prenatal patients delivered at the hospital and women with abnormal gynecological exams received timely comprehensive follow up and treatment.
