“How difficult can it be?”. I am intrigued by this statement, that we hear so often after an unexpected event. I therefore set out to reconstruct the context in which it made sense for the people to do what they did leading up to the event, on all organisational levels. The reality of work is often far more complex than we all initially realise and I find a holistic approach to be essential in discovering what enabled the event and what the can be learned from it to prevent reoccurrence.I apply this approach in the investigation of serious incidents and accidents involving aircraft in the general aviation, commercial air transport and military domains. Furthermore, I am a Private Pilot (PPL), a member of the Dutch Air Force Reserves (GLR) and I have an academic background in aviation engineering, safety and human factors.

The investigations of the board are intended not only to ascertain contributing factors but also to consider systemic issues that have an impact on safety. The purpose of all Board investigations is to learn from the accidents and to make recommendations in order to improve safety. Its investigations do not address issues of blame or liability.

The position was based on a 24/7 on call shift system divided among 4 crisis coordinators.
