At the Onderzoeksraad voor Veiligheid (OVV)/Dutch Safety Board (DSB), I investigate serious incidents and accidents involving aircraft in the general aviation, commercial air transport and military domains. The investigations of the board are intended not only to ascertain contributing factors but also to consider systemic issues that have an impact on safety. The purpose of all Board investigations is to learn from the accidents and to make recommendations in order to improve safety. Its investigations do not address issues of blame or liability.

At the Onderzoeksraad voor Veiligheid (OVV)/Dutch Safety Board (DSB), I advised the accident investigation teams on topics such as accident analysis methods, investigation focus, safety evolution, interviewing techniques, investigator biases and system safety in order to assure the quality of the investigations. Furthermore, I performed Human Factors analysis as part of the investigation teams.

Air Force Headquarters, Breda, Netherlands

I worked part-time (1 day a week) at the Royal Netherlands Air Force (RNLAF) as a reserve officer (Groep Luchtmacht Reserve: GLR), assisting the RNLAF safety department in the investigation of incidents and accidents.

Schiphol Airport

At Air Traffic Control the Netherlands (LVNL), I performed incident investigations and safety analyses at the Operational Risk Management (ORM) departement. My main focus was ATM safety of the regional airports (EHGG and EHBK) and Human Factors investigations for incidents at Schiphol Airport (ISMS).As a Crisis Coordinator, I assessed the severity of a calamity or crisis and the impact to LVNL, in addition to my role as a Safety Expert. If needed, I then coordinated between the LVNL board level crisis management team, partners and other crisis teams. I was responsible for the process during crisis team meetings. The position was based on a 24/7 on call shift system divided among 4 crisis coordinators.

Schiphol Airport

As a Human Factors Expert at Air Traffic Control the Netherlands (LVNL), I applied a human-centred approach to design and development of the ATC system (systems, routes, procedures, training, culture, etc.) and corresponding training of air traffic controllers.

Utrecht Area, Netherlands

As a rail accident investigator I performed several on-site and off-site investigations following a railway accident, together with a team of technical or operational specialists. The investigation resulted in a report with the facts, analysis, conclusion and recommendations for management. I had a coordinating role in the accident investigation team.

Amsterdam Area, Netherlands

Guest lecturer Human Factors & Aviation Safety at the department of Aviation Studies at The Hogeschool van Amsterdam, University of Applied Sciences (HvA). Topics included systemic failures, safety culture, Safety Management Systems and accident analysis.

Soesterberg

The research topic of my Master of Science (MSc) thesis included a quantitative evaluation of the Royal Netherlands Air Force (RNLAF) Crew Resource Management (CRM) training at Centrum voor Mens en Luchtvaart (CML), Soesterberg.The final report was titled "The effect of RNLAF CRM training on participant attitude and retention over time".
