Case Manager Defined: A range of client-centered services that links clients with health care, psychosocial and other services to insure timely, coordinated access to medically appropriate levels of health and support systems, continuity of care, ongoing assessment of the client’s and other family members’ needs and personal support system, and inpatient case management services that prevent unnecessary hospitalization of that expedite discharge, as medically appropriate, from inpatient facilities.On-site case manager duties:• Links client with health care services psychosocial and other services, as needed and requested by the Palm Beach County Health Department clinic staff.• All activity conducted that includes services and referrals.• Agency acronym by use of an ink stamp and date and case manager signature.• Assist clients with applications for any type of health insurance, including Medicaid, Health Care District etc. and documenting in medical record.• Require client to provide a copy of denial letter of application of any type of health insurance to be placed in medical record chart before accessing payment through RW, which is the payee of last resort.• Serve as client’s advocate when appropriate.• Assist client with adherence and follow-up adherence issues.Specialties: I am currently a board member of the Palm Beach County Equal Opportunity.
