I am responsible for the vision and direction of Advanced Practice within the Trust, across specialities where Advanced Practitioners are deployed.I have corporate responsibility for the assurance and governance of Advanced Practice, engaging with internal and external stakeholders to ensure we are maximising the full value of Advanced Practice to patients, clinicians and the Trust alike.I deploy at least weekly (often more) throughout our Emergency Operations Centre, SP/ACP RRV or within the Trust operated Urgent Treatment Centre. In addition I oversee the Trusts ACP on-call function, which is designed to offer internal senior clinical advice.

Joint line management and clinical supervision responsibilities for the clinical team alongside the PCN Manager.I am responsible for implementing and delivering the clinical components of the PCN DES contract and IIF across a footprint of 5 practices.Clinically I practice as an Advanced Clinical Practitioner with an emphasis on primary and urgent care. My current practice sees me having overall clinical responsibility for our Enhanced Health in Care Homes workstream, with a focus on comorbidity and frailty for a bed base of 460 patients.Following the development and successful implementation of the PCN Clinical Strategy, I was able to introduce rotational clinicians who deliver the urgent visiting service; urgent care clinics and home rounds.One key success was the introduction of QoF reviews for housebound patients, who previously seldomly received an annual review.

Working as a senior ACP I provide clinical care to patients presenting with urgent health care needs over the out of hours period. I provide remote triage, ambulance and emergency department revalidation and offer face to face review in the treatment centre or patients own home.I am also available to provide clinical supervision to Home Visiting Paramedics and First Contact Practitioners.

Direct line manager for Nurses, Paramedics and Clinical Pharmacists working within the Integrated Urgent Care Service / Out Of Hours, ensuring staff are supported and developed to enhance the service and level of care to our patients.In the background I work on a variety of projects and work streams, requiring me to prioritise and manage my work load to ensure timely completion. I attend clinical cabinet, significant event and quality assurance meetings, providing my clinical knowledge and acumen to support these where I can.

Leading the Urgent Care Team I am responsible for the day to day running and management of the non-medical workforce consisting of Nurse Practitioners, Advanced Nurse Practitioners and Paramedic Practitioners.Utilising AskMyGp I undertake a sorting session each morning deciding the most appropriate course of action for acute and urgent care presentations. Contacts can be managed through message, telephone triage, face to face assessment or home visit. Each request is triaged and streamed to the appropriate team member.I am currently undergoing development in HR functions of leading the team.

Working as part of the ACP led Urgent Care Team I am responsible for managing patients with undifferentiated illness or injury, long term conditions and acute on chronic presentations. I undertake assessment and management through face to face consultations, telephone triage and on home visits.I independently assess, diagnose, discharge and refer as appropriate, initiating prescribing decisions where needed.As a special interest I am also responsible for delivering the QOF components for cardiovascular health and hypertension from initial diagnosis.

I am responsible for facilitating CPD events for post registration AHPs and supporting the education of both under and post graduate students within primary care.The role is still in development, as the training hub has only recently been created.

I work autonomously as a senior clinician offering enhanced patient assessment skills and management strategies to better care for patients at the point of injury or illness. I am able to accept crew referrals and offer advice to colleagues to offer the latest evidenced based practice approach to the patient journey. Working at a senior level allows me to avoid hospital admissions reducing the burden on both the ambulance service and acute trusts.Having moved into primary care I now work for SWAST on a bank basis, ensuring my emergency skills are kept current which can transfer into my practice within the medical centre.
