Final Year Project
Intracranial aneurysms are a disease of the brain vasculature. They appear as a sac-like out-pouching of a part of the arterial wall, inflated by the pressure of the blood. They are relatively common and affect up to 5% of the adult population. Most remain asymptomatic; however, there is a small but inherent risk of rupture. If rupture occurs there is a 30% to 50% chance of fatality. Consequently, if an aneurysm is detected, clinical intervention may be deemed appropriate. However, interventional procedures are not without risk to the patient. Given the relatively low risk of rupture (0.1 to 1% per year) it would be desirable to be able to identify those aneurysms most at risk of such an episode. This would assist clinical diagnostic procedures and avoid the potentially undesirable consequences of an unnecessary operation.Aneurysms develop in the size as the tissue remodels. The vascular cells that remodel the tissue are informed by clues from their mechanical and chemical environment. Computational models of aneurysm evolution that represent the tissue and the cells and the forces that they are subject to (due to pulsatile blood flow) will guide our understanding of the disease and used to inform clinical decisions, i.e. to treat or not to treat. This project aimed to improve the current model by connecting mechanical and chemical stimuli related to aneurysm stability.
Research Internship
Mechanical thrombectomy is currently the principal treatment for ischaemic strokes, where thrombi are removed from the patient’s cerebrovascular system using a device (Figure 1) consisting of a guide catheter, stent and balloon. Functionality and behaviour of blood flow such as direction or speed are normally unknown until deployment of these devices; and the insertion of the catheter further influences and changes these as result of a reduction in area and increased internal pressure. An analytical solution was formulated, which provided insight into the resistance inside blood vessels when a catheter is inserted which theoretically could affect overall haemodynamic behaviour, and the purpose of this research was to validate this using 3D CFD models of cerebrovascular vessels.
