My role as a Community Outreach Case Manager is to bring quality healthcare services of the clinic to underserved populations by building bridges between the patient and the provider.

Provided case management and ADL services to SMI, CA and SED clients and their families. These services were directed toward: providing the skills and supports necessary to enable clients to function in the least restrictive environment with the highest possible level of functioning. Case Manager provided assessment, service planning, coordination and linkage and monitoring functions.Assisted with locating those that are homeless with mental health issues and developing trust and built relationships with these individuals. Screening and intake, along with linking to CMH and mainstream resources. Data entry into HMIS (Homeless Management Information System) and utilized VI-SPDAT (Vulnerability Index - Service Prioritization Decision Assistance Tool), to assess their needs.

I worked directly with consumers in nursing homes throughout suburban Cook County who were seeking affordable and accessible housing as part of their reintegration into community through our Community Reintegration (CR) program. I assisted consumers in locating housing, completing housing applications, and providing skills training.

Responsibilities include case management; completing assessments; developing, implementing, and coordinating meetings for care plans in conjunction with the interdisciplinary team; counseling; running therapeutic and skills training groups; facilitating communication between families, psychiatrists, and the facility; advocating for resident’s rights and well-being; developing and maintaining the residents incentive programs; discharge planning; accompanying residents on appointments; following all Illinois Department of Public Health regulations; and crisis intervention.
