Medical Social Worker
Current-Check on the progress of applications for patients (typically Medicare, Medicaid, and disability applications) to inform patients and/or caregivers to understand next steps.-Collaborate with other organizations to develop relationships and obtain referrals for patients, which creates relationship-building and community-building needs.-Help patients with financial resource constraints to ease their burdens.-Apply for additional government support needs on the behalf of patients.-Inform caregivers of behavioral issues within family systems of different diagnoses. -Advocate for patient medical needs by spreading disorder awareness on various platforms.-Help with patient placement to ensure long-term care. -Initiate community outreach to support patient and family needs and well-being.-Provide initial assessments for patients to understand their history/needs. Encompasses the whole family system, which includes family dynamics and family history.-Administer IDT for the team to review patient notes, which involves various nurses, admission nurse, hospice doctor, office manager, and the Administrator Chief Operations Officer.-Collaborate daily with nurses to meet patient needs to make sure patients are routinely cared for, ensuring high quality of care.-Advocate on patients’ behalfs by researching healthcare, transportation, housing, clothing,technical issues, and financial services.-Provide miscellaneous services depending on resource needs.-Provide companion care to accommodate additional needs.-Facilitate Art 4 Healing workshops as a therapeutic technique to increase patient overall well-being.
