This senior pharmacist role involved specialist rotations in governance, anti coagulation, surgical, clinical trials, high dependency/critical care and antimicrobial pharmacy, in addition to my previous role's (band 6) I have also taken on many further responsibilities.I have completed 9 months in governance since I started this role.My duties include:-Working with Medway CCG to approve the funding for use of high cost drugs within the Trust.-Attending DTC meeting to approve new drugs for the formulary. -Working as part of a committee to approve PGD use within the trust.-Compiling audit reports to feedback ward performances to ensure standards.-Working with CQC to ensure recommendations are actions within the trust.-Being the handler of "datixes" within the Trust, and implementing an action plan to prevent recurrence.-Training pre-registration and newly qualified pharmacists on the wards.-Designing and implementing a Pharmacy newsletter to ensure fellow healthcare professionals are up to date with MHRA alert and changes to pharmacy based processes within trust.-Working on an admission ward with consultants, junior doctors, specialist nurses, to ensure effective pharmaceutical care for inpatients.-Training pharmacists newly qualified pharmacists to provide an out of hours service- Writing, reviewing and implemented clinical guidelines for use within the Trust.I am currently on the anticoagulation specialist rotation, and at the moment my duties include:-Carrying out training on safe prescribing, monitoring and rationale for initiating anticoagulant treatment for fellow healthcare professionals.-Dosing warfarin for inpatient and community patients.-Providing clinical advice for community and trust prescribers.

This job was a rotational training programme to develop my skills required as a pharmacist in all areas of clinical pharmacy. I rotated between various departments including distribution, dispensary, clinical wards, medicine information, community pharmacy and aseptic preparation services.My general responsibilities include:• Labelling, dispensing and screening prescriptions• Answering telephone queries• Medicines reconciliation• Patient returned controlled drug destruction• Pre-validating patient discharge medication under instruction.• Counselling inpatients and outpatients on new medication and medical devices.• Working on the late night and weekend rota when required.• Supervising placement students.• Advising other healthcare professionals on drug information and prescribing advice.• Safety and efficacy monitoring of drug therapy.I am also medicines reconciliation accredited, have carried out a clinical audit on warfarin counselling and also completed a 200 item dispensing and accuracy checking log.

My responsibilities included dispensing different types of prescription forms and checking the prescription for legal and clinical errors (i.e. interactions, dosing, allergies) and where necessary reporting such errors back to the prescriber. As well as this I had a responsibility of offering medical advice for minor ailments to customers, and when necessary advising on the best course of treatment for the patient.I also prepared dosette boxes for elderly patients or those on many regular medicines, to aid patient compliance. I also prepared methadone doses using the CD register as well as supervising the dosing of these patients.During my time with Boots, as a pharmacy student, I learnt how to dispense prescriptions efficiently under pressure, respond to symptoms and provide safe over-the-counter treatments for patients. Most importantly I learnt of the importance of communication and time management skills, and the importance of complying with standard operating procedures to ensure the highest level of patient care.My duties overall included:• Responding to symptoms and suggesting best course of action.• Screening prescriptions, and reporting my findings to the responsible pharmacist.• Training and supervising new staff.• Dispensing and labelling prescriptions• Arranging repeat prescriptions with surgeries for patients.• Stock management and procurement• Counselling patients on newly prescribed medicines and medical devices.• Recording fridge and storage temperatures.
