IPAR reviews: I worked with the pharmacist for IPAR reviews (In-Patient Admission Review) to create a BPMH (best possible medical history) for new long-term care residents. Depending on where the patient was coming from I would collect medication history/medication administration review from the hospital, community pharmacy and the patient or their family member. After confirming a patient's most recent medical list, the pharmacist and I would thoroughly review the doses, indications, frequency and drug interactions. The last step of the IPAR review would then be to contact the physician of the long-term care home and get an approval order for the medication list. Narcotic and Cytotoxic Medication Check: Every day of the week, our pharmacy would send narcotic and cytotoxic batches to different long-term care. I would thoroughly check for any errors ensuring the correct medication was for the correct patient, the correct home and the correct amount was being given on the specific days as per the signatura. I would then double-check with the auto-med and narcotic check report before signing off and packing the narcotic and cytotoxic batches to be delivered to the home.Refill requests: Each home had a system for requests for refills. I would enter the Rx number of the patient's name on Kroll to confirm that the patient was indeed due for a refill. If the patient was due for a refill, I would authorize one, if not, then I would send a notice to the home to send another request writing out when the date of the request should be and how early their current request is.Signing off on refills: After the refill request was authorized and filled by our pharmacy technicians, I would perform prescription checking to ensure that the current drug was filled and labelled for the correct patient and home. I would then sign off on the hard copy and send it to be delivered to the respective home.

Shopper's Drug Mart: 3035 Appleby Line was my placement provided by the University of Toronto for my first placement after my first year of pharmacy school. My preceptor was Baseer Yaseen. In this placement, I learned the complete process of how a prescription comes into how the medication is dispensed to the patient.- Input a new prescription. Confirm the patient identifiers (Name, DOB, Address and Phone number) and any allergies or new allergies. If the patient is a child also confirm current weight. Lastly, if the patient is being prescribed an antibiotic ensure to collect indication. -Process the prescription on the correct patient profile and input the correct drug, sig, mitte and repeat. The pharmacist will check the processing before allowing to fill.-Fill the drug and label it. Make sure the medication is correct and has not expired or been already opened. -The pharmacist will check the filling before packing for pick-up-When the patient comes for pick, you confirm their name, address and date of birth before dispensing the medication. Appropriately counsel the patient when necessary and add counsel notes if needed. I counselled primarily on antibiotics, eye and ear drops, topicals and inhalers.Along with learning and participating in the process outlined above I also participated in Drug Information questions by patients with active help from the managing pharmacist. Lastly, I performed 4 medscheck and listened in on two diabetes medschecks.
