Community rehabilitation unit.

Band 6 - Community Crisis team older adults. Working predominently with older adults but some cross working with working age patients as well. Responsible for triaging new referrals, completing assessments, monitoring core symtoms and managing risk. Providing support, assessment, treatment and intervention for patients as well as liaising with other professionals and providing support for carers. Using good communication and documentation skills for all care and contact provided. Using good situational awareness and risk management for working within individual's homes. Providing support and education to student nurses and junior staff. Keeping up to date with policies, procedures and ensuring working from a least restrictive, evidence based and recovery focused perspective. Engaging in reflective practice and supervision to maintain personal wellbeing as well as utilising oppotunities for further training to improve knowledge and skills.

Band 5 staff nurse - Inpatient organic assessment unit. Working as nurse in charge on busy dementia ward. Shift coordination, managing and delegating tasks to junior staff. Providing support and directon to others and supporting learning of student nurses. Medicaton management, ordering and administering as well as completing relevent audits. Working from a dementia friendly needs led perspective, completing forumulations and sharing information with staff and professionals. Delivering information in teaching sessions in aged care facilities to facilitate effective transfer of care and successful discharge. Managing behaviours which challenge utilising least restrictive interventions. Developing person centered plans of care and liasing with other professionals and family/carers. Working with a variety of different professionals and leading meetings to discuss care and handing over daily. Maintaining knowledge of best practice and of relevent policies, mental health act and mental capacity act etc. Completing assessments of mood, mental state and cognition. Managing physical health and escalating concerns appropriately in cases of deterioration. Completing additional training in communication and care planning to ensure meeting patients needs and improving outcomes.
