Peer Navigator
Current•Enhanced Care Management (ECM) medical case management for people experiencing homelessness in Garden Grove. •Community Supports Services (CSS) housing navigation and support services for people experiencing homelessness in Garden Grove. This includes assisting in gathering of vital documents needed for housing like birth certificate, identification card, and social security card. •Managing a patient load of 25+ patients while doing both ECM and CSS case management. While being able to make at least 1 in person visit with them per month and 1 telehealth visit for every patient. •Assisting patients manage their medical needs by helping make and manage their appointments, this can be through paper reminders, phone calls, assisting them with transportation, or accompanying them to their various medical appointments or related appointments. •Assisting patients with medication pickup, storage, and weekly dispensing in cases where extra assistance is needed. •When necessary, accompanying and advocating for patients in the hospital settings to ensure the highest level of care and fighting against stigma placed on unhoused individuals. Speaking to doctors, nurses, social workers, and hospital staff that will have anything to do with patient treatment plan. •Learned to navigate the Orange County Homeless Management Information System to assist our patients with housing navigation and appropriate housing options based on opportunities and their preferences. •Attending weekly housing match-making meetings for the service planning areas where patients have housing opportunities run through the Orange County Homeless Management Information System (HMIS).
