Program Manager
Current• Responsible for overseeing all aspects of Support Services assigned to their caseload and ensuring health and safety of supported individuals age range 40yrs to 72yrs, as well as overseeing their growth and development.• Participation in the development and implementation of the Individual Program Plan and Individual Service Plan, monitoring the plans, and assisting with revisions as necessary.• Assisting consumers and their families in acquiring and maintaining assessed supports and services; serving as an advocate for the consumer with community agencies.• Participation in the development of the Individual Program Plan (IPP); monitoring the IPP and assisting with revisions as necessary; assisting consumers and their families in acquiring and maintaining assessed supports and services; serving as an advocate for the consumer with community agencies; consulting with and assisting vendors with certifications, applications and referrals; completion of all required forms, documentation and report• Locate and coordinate services. Provide technical support and direct assistance in locating services to include resolving crisis situations. Monitor identified supports and services to determine satisfaction with services, addressing concerns as necessary.• Advocate for and assist supported individual in advocating on their own behalf. Participate in activities that promote community awareness and acceptance of people with disabilities. • Assist supported individuals in identifying life goals and specific preferences. Assist supported individuals in identifying and utilizing community connections and natural support networks. • Educate individuals about self-determination, enviable lives, and assist individuals in exploring the implications for themselves.
