Providing Applied Behavior Analysis (ABA) services for autistic children, to help support them in their social, communication, emotional, adaptive, and academic skills.

Essential Duties Completed:- Facilitated admissions and secured documentation of 125 new participants with the agency in CILA, Day Program, Intermittent CILA, and Home-Based Services.- Gave presentations on UCP Seguin's services at three professional conferences to families of individuals seeking disability services (The Arc of Illinois, Service Inc)- Managing a tracking list on referrals of over 550 individuals seeking services.- Conducted 60 screening interviews with individuals/families seeking services.- Attending resource fairs and house tours to promote UCP Seguin.- Facilitated the admission of 10 participants to receive services due to emergency reasons.Administrative Tasks Completed:- Made comprehensive updates to intake and annual consent forms to provide to guardians/families.- Faciliated creation of a full list of Spanish-language versions of the agency's consent forms.- Developed a method for the agency's nursing department to obtain PDF versions of consumer medication lists from their collaborating pharmacy in accordance with HIPAA.- Created fillable PDF versions of each of the agency's consent forms to distribute to families/guardians.- Trained over 50 staff and professionals with data entry for CILA residents on HSMS.- Aided another case manager with completing annual assessments for 15 participants.- Updated the consumer handbook to be distributed to individuals receiving services.- Attending monthly conference calls with a Multi-Disciplinary Team (M-Team) to address abuse/neglect cases with Adult Protective Services (APS).- Created a fillable PDF version of the agency's COVID-19 screening tool for all employees to access.- Updated benefits and evaluation information of over 500 participants in the agency.- Created a presentation on rent policy/process for consumers who are CILA residents of the agency.

• Scheduled intake psychiatric appointments for three participants to ensure professional care.• Actively advocated for the medical well-being of three participants during their hospitalizations.• Renewed Pace ADA Paratransit certifications for two participants.• Aided an individual to move to a new high school, and in securing/maintaining a job.• Independently advocated for four special staffings to address participant issues.• Arranged for a participant to transfer to a new day program.• Successfully advocated for a participant to receive wheelchair repairs.• Completed applications to ensure two participants attend SRCA summer camp.• Coordinated transportation for a participant’s medical appointment.• Conducted on-call visits for three participants during their psychiatric ER visits.• Secured case management support for five DT-only participants.• Updated a participant’s service plan to assist a guardian in obtaining an AAC device for her son.• Worked weekend DSP shifts at a group home for two months.• Advocated for the reduction of a participant’s medication, resulting in fewer ER visits/hospitalizations.• Advocated for two participants to receive medical/nutritional support to maintain/gain weight.

The IIT Center for Research and Service is currently under a contract with the current Chicago Public Schools system and maintains a center of selective enrollment testing for applicants to standard and gifted elementary schools.• Gave testing center instructions to an average of 200 families per day to the Chicago Public Schools Selective Enrollment testing program at IIT.• Established positive rapport with students from grades PK-8.• Confirmed appointments and answered questions from families.
