To provide a Occupational Therapy service to professional standards, with patients with diverse presentations. To provide a clinical diagnosis and develop & deliver an individualised treatment programme. Working as part of a multi disciplinary team within the community setting, within Northamptonshire.

Completion of full Occupational Therapy Functional Assessments, within Warwickshire County Council's 'Housing Assessment Team'. Visiting service users in their own home environment's, assessing their 'long term' needs for Personal Care, Access Issues, Mobility & Transfer difficulties.Provision off Disabled Facilities Grants for major adaptations for both.

Northampton

Assessment off forensic Mental Health patients, within a locked ward. Being aware off risk at all times.
