• Work with foster carers and case managers in supporting children and young people in placement.• Support carers with behaviour management and contribute to resolving issues that may cause placement breakdown. • Case load involved with this are all of Shropshire council’s foster carers and children or young people in foster care.• Contribute to the delivery of care support programmes alongside professionals.• Develop monitoring schemes in conjunction with Team Managers, Children’s Placement Staff and others to help gage the effectiveness of support in terms of outcomes for foster carers and children. • Use the database Care First to report the outcomes of foster support.

• Take details of young people coming into care and ensuring a referral for each young person is presented alongside their admission. • Where appropriate obtain essential information which is required by the Departments Mandatory Data Set. • Once the referral is submitted then asses the urgency of the circumstances of the child or young person and seek to identify an appropriate placement in accordance with their needs. • Ensure that all referrals received are screened by the Duty manager and all actions attached to the referral are approved/ authorised appropriately. • Keep a record of every young person’s/ child’s referral that is received and the outcome that is achieved, through written accounts and computer data. • Use clear lines of communication with a number of professionals involved in the safeguarding team. • Liaise with fifty + agencies, both internal and external, of Shropshire council in order to meet each child or young person’s individual needs.

Consistently develop skills through training in areas of work relevant to the setting.

Followed 2 cases of children from admission into Foster care all the way through to being placed with Connected Person’s and long term foster care. This Case involved • Shadowing several meetings such as; CORE Groups, Looked after Children’s Reviews, Supervised contact and Personal Education Plan meetings. The meetings involved open and effective communication from all professionals involved with the child and family including; health visitors, foster carer’s, teachers and advocates for family members. • In conjunction with these meetings I have contributed to each child’s Care Plan. • The Care Plan assembles all the information provided by the previously mention meetings in order to collate a plan which best suits the needs of the child. • Contributed the composition of a court report relating to obtaining initially an Interim Care Order and then Full Care Order under the Children Act 1989. • During this time I was also shadowing the connected person’s assessment process through to the date of placement.
