My role involves investigating allegations of elder abuse, neglect, abandonment, and financial exploitation. I am committed to offering services that detect, reduce, correct, or eliminate risks associated with various forms of abuse identified during investigations.Key Competencies:Investigation and Assessment: Skilled in receiving and investigating reports of abuse, neglect, exploitation, and abandonment of individuals aged 60 and above. Proficient in determining the substantiation of reports and assessing the need for emergency interventions.Case Management: Experienced in completing investigations and developing care plans for older adults, ensuring comprehensive support that may involve family members, friends, and formal service providers. Provide ongoing case management in accordance with the Older Adult Protective Services law.Documentation and Compliance: Ensure accurate, timely, and relevant record-keeping in adherence to State regulations, agency policies, and best practices. Comply with departmental performance benchmarks and regulatory requirements.Collaboration and Coordination: Establish and maintain relationships with public service providers, including mental health and healthcare professionals, police district offices, and other community agencies. Coordinate care planning with various service providers and delivery systems.Resource Mobilization: Proficient in locating and mobilizing resources to meet the diverse needs of vulnerable older adults. Identify service gaps hindering the provision of high-quality care and work towards solutions.
