As the Director of Population Health Management I was responsible for the development, implementation and management of Disease/Population Health Services leading to improving the quadruple aim (ie. Improved health outcomes, improved patient and provider satisfaction and decreased total cost of care). I served as a leader in driving clinical transformation, delivering a comprehensive interdisciplinary approach to achieve care delivery excellence throughout the patient care continuum that measurably improves quality, creates holistic, patient centered care experiences, and reduces healthcare costs by reducing waste and optimizing the value proposition. I oversaw the operations of our Specialty Clinic (ie. As Vice President of Clinical Integration, I am responsible for developing clinical programs that enhance the performance of an at-risk integrated delivery network (Sequoia Integrated Health), particularly in the areas of preventive care, chronic care management and medication adherence. I am responsible for the development, implementation and management of clinical integration initiatives, while further developing relationships between and among the physician group, hospital, health plan, administration, and the respective quality improvement programs. My role is to be a leader in driving clinical transformation that integrates demonstrable, value-added, interprofessional, patient-centered care to the patients in the region. I provide executive oversight and leadership for providers, ensuring providers are in the forefront of innovation and delivery of high value health care. I'm a collaborative team player who actively participates in the development and execution of the strategic goals, objectives and plans of the organization while supporting and enhancing partner initiatives in improving clinical quality, patient safety, and patient experience. I also serve as a member of the board of directors for SIH.

Founded a 501c3 non-profit organization and helped form an 11-member board of directors (6 patients and 5 community members) to form a public entity co-applicant federally qualified health center (FQHC) to support and expand the organization's ability to provide and expand access to high-quality primary, behavioral and specialty care to the urban areas of our underserved community. I oversaw the formation of the corporation and its bylaws, the co-applicant agreement with the public entity, as well as policy creation and implementation to ensure full compliance with HRSA health center compliance manual. Oversaw the creation and submission of applications for both New Access Point and Look-Alike designation. Responsible for preparation for HRSA's On-Site Visit (OSV) and resolution of findings. Participate on all Board Committees (Finance, Quality, Strategic Planning, Executive).
