-Assisted low income individuals in locating and applying for health and social resources.-Developed a resource binder with social and healthcare related resources in the Savannah-Chatham County area.-Designed literature and test result information for patients at low literacy levels.-Assisted patients with applications for technical school, resume creation, and job application.-Conducted patient interviews to assess patient need for social resources, and facilitated patient application for resources.-Visited patients in their homes, and assisted these patients in improving quality of life, including delivering food, and fixing a terminal cancer patient’s television.-Continued with the program as a volunteer after graduating, serving as a supervisor for a student, developed with a patient-focused pamphlet of food, clothing, and shelter resources.-Mentored three students who entered the program at a later date. Connected the students with patients and resources.-Utilized health education techniques including the Teach-it-Back method and motivational interviewing.
