Graduate of Our Lady of The Lake University for NonProfit Management Business DegreeServes as the Transition liaison for the cluster to establish relationships with families, teachers, andcommunity agencies that support students ages 3-21. Communicates information to parents regarding relative community events and parent trainings.2. Schedules and facilitates inter-agency meetings with adult students and high school parents.3. Schedules and facilitates annual Transition Fairs for students and parents of students with disabilities.4. Establishes and maintains a network of community resources to include local universities, colleges, and employers.5. Coordinates instruction and evaluation of student performance with other North East Independent School District (NEISD) staff and personnel.6. Collaborates with district staff in preparing for Admission, Review, and Dismissal (ARD) /IndividualEducation Plan (IEP) meetings when information pertaining to transition and vocational activities/outcomes will be discussed.7. Attends and participates in meetings (Admission, Review, and Dismissal (ARD), Department, Grade Placement Committee (GPC), etc.).8. Facilitates the auditing process for State Performance Plan (SPP) Indicators 13 and 14.9. Facilitates and provides districtwide training pertaining to transition, including legal framework, community resources, vocational and transition curriculum, job coaching, etc.10. Consults with campus personnel and provides support for work-based learning (WBL), community-based vocational instruction (CBVI), and pre-vocational programs for students with disabilities.11. Consults with campus staff and personnel regarding the development of appropriate transition plans and personal graduation plans (PGP) that ensures positive post-secondary outcomes for students withdisabilities.13. Monitors and provides support for students with disabilities enrolled in Vocational Adjustment and Career and Technical Education (CTE) programs.
