PACE stands for Program for All-Inclusive Care for the Elderly. As an Occupational Therapist with PACE, I conduct evaluations in the clinic and in participant homes for many different reasons - to determine a potential participant's appropriateness for the program, to determine a participant's need for Occupational Therapy services (every six months, following a fall, following a change in status, or in response to a referral from another team member), to determine a participant's need for DME or home modification.I also manage a caseload by conducting participant interventions and work with an amazing COTA. I communicate continually with interdisciplinary staff regarding participant needs. Currently I am working with an interdisciplinary team for submission of a Falls Prevention Grant.
