Autism Peer Specialist
As an autism peer specialist, I provided peer support to adults with autism by means of educating, informing, mentoring, and motivating to help and support them pursue their goals and objectives. I have conducted weekly, bi-weekly, and monthly check-ins with my individuals via telephone, email, and Zoom. I have facilitated skill building and establishing relationships with individuals by running focus groups such as Technology group and Poetry group. When appropriate, I would share lived experiences to help adults with autism think about their personal living situations to generate decision-making through autonomy and self-empowerment. Home Care Case Manager
As a home care case manager, I used to do the following: Conducted 3QVs, 6QVs, and Annual visits for consumers; wrote narratives based on the visits conducted to evaluate and analyze the consumers' services, their health status, and whether any recommendations should be made (i.e. referrals to different programs within the agency; falls prevention; evaluations for specific medical concerns such as blindness); followed up with providers and vendors to increase or decrease consumers' services for homemaking, personal care, PERS (personal emergency response system); adding service suspensions when consumers were in the hospital or in rehab; Delivered HDMs to consumers (when necessary) due to their individual circumstances when Meals on Wheels does not drop of HDMs (home delivered meals). I would also attend in-service meetings to learn about new resources, topics, and information relevant to working with older adults. I also participated in informational interviews with the providers and venders to become more aware of the consumers' respective communities to understand what resources, supports, and services were available.
Clerical Receptionist
In March of 2018, I was reassigned to the Clerical department as a Clerical Receptionist due to the fact they needed extra coverage for their department, and I was willing to support the needs of the agency and the department. When I started with the Clerical department in March until I finished my employment with the agency on January 11, 2019, I was helping out with responding and/or transferring incoming phone calls from consumers and vendors to the appropriate personnel using a multi-line telephone system. I also assisted with the stamping and distribution of mail for employees and personnel. I completed some faxing and filing of PSI forms for the Senior Care Options (SCO) and State Home Care (SHC) programs as well as file State Home Care (SHC) and One Care paperwork in consumer files. I helped out with completing key inventories for various departments to make sure employees have keys for their doors, desks, and cabinets that work (i.e. SHC department, SCO department, PCA department). I would also provide assistance with stocking of supplies for both the lower and upper floors of the agency. I assisted with assembling closed cases in manila folders for consumers no longer receiving services here at MHCC and file them in the cabinets with appropriate sections (i.e. Deceased, Other, Nursing Home) and the appropriate year. I also collaborated with other departments with assignments and projects when they need extra assistance with permission from the clerical supervisor (i.e. Completing approved extra hours to help the SHC department with completing telephone assessments as well as help the department with scanning and uploading risk planning, mini cog, and falls screening worksheets when necessary). Finally, I assisted with locking up clerical department cabinets and desks; money box; log in/log out book; supply closet; and mail cart prior to closing of the office for the day.
Shc Case Aide
When I was first hired as a SHC Case Aide in November of 2017, I was assembling cases for consumers to be opened by supervisors involved in the State Home Care program. In addition, I was conducting phone assessments for consumers that are due to have them to address and/or update medical information; health status; any falls, hospitalizations, and ER visits within the past three months; formal and informal supports; activities of daily living (ADLS) and instrumental activities of daily living (IADLS); care plans and units/hours of homemaking and/or personal care; and any follow up to case managers that is necessary upon the requests of the consumers. I was also coordinating and scheduling supports and services with vendors to address the needs of the consumers such as providing Lifeline PERS; homemaking; and personal care.
