Provide information about Employment Services over the phone, in-person, and via e-mail to Centre customers, community partners and the general public. Register new customers and assist them in completing applicable paperwork. Schedule clients for appointments with Employment Advisors and register clients for workshops. Support clients in conducting web-based job searches, troubleshoot basic computer-related problems and format job search documents. Provide customers with appropriate information related to various community-based programs in Halton and surrounding area. Enter client information into The Centre’s Client Tracking System (CTS) database and in CaMS.

During the placement, which was both in the ICU and Surgical Units, drew upon the expertise of the entire inter professional team and collaborated with the relevant team members when assessing and developing a client centred nutrition care plan. Also, researched and developed a tool for managing blood sugars in nutrition support patients. Had the experience of working with a variety of clinical conditions as sepsis, renal failure (with and without dialysis), stroke, sub-arachnoid haemorrhage, major GI surgery (Whipple's, hepatic resection, bowel resection). Developed and provided education to patients with a variety of medical conditions like diverticular disease, cholecystitis, diabetes, colostomy, high energy/high protein. Attended and participated in daily inter-professional rounds both in the ICU and the Surgical units. Exhibited strong critical thinking skills and was able to utilize this skill in assessing some very complex patients.

Conducted one-on-one nutrition counselling sessions on various topics ranging from anemia, healthy plate model, weight gain/weight loss, high fiber diet, healthy lifestyle approach, hyperlipidemia, hypertension with a professional client centered approach and establishing good rapport with the clients. Researched and developed a 2 hour workshop "Sugar, Sugar" including facilitator's manual, game, quiz, worksheets, power point presentation, relevant videos and handouts along with pre and post questionnaire forms to evaluate the workshop. Facilitated two workshops on Canada's Food Guide at partner community centers for senior citizens and for adult new immigrant women. Organized the Dietitian's booth for two Healthy Living Events hosted by AAMHCS focusing on diabetes prevention with healthy eating and reduced sugar consumption. Organized and conducted training sessions geared towards nutrition student volunteers, peer leaders and staff liaisons for the Newcomers Cooking Together (NCT) program.

Explaining to new clients about the services at the center, registering new clients in the computer system. Scheduling appointments of the clients with the employment advisers and for the workshops. My responsibilities included taking history, performing physical examination, requesting lab. tests, diagnosing and prescribing medicine to over 40 patients a day. Evaluating and then designing a personalized meal plan for the adult clients (both male and female), and help them establish realistic goals in improving their nutritional status. Providing tailored meal plans to patients having chronic illnesses as high blood pressure, diabetes and heart diseases.. Discussing complicated cases with the senior doctors for the evaluation of patient’s health and proper diagnosis and treatment. Advising parents about the immunization of their children and administer the vaccines to the infants and young children. Giving presentations at different forums for the purpose of health and fitness awareness among the general population
