Retinopathy of Prematurity (ROP) is a major cause of visual impairment worldwide, resulting from high levels of inspired O2 used to compensate for lung underdevelopment in premature infants. The initiating event in ROP is delayed retinal vascularization - an adverse effect of high O2 levels in the retina due to O2 supplementation therapy. As photoreceptors are depolarized and consume more O2 in the dark, we hypothesize that this side-effect of O2 supplementation can be mitigated by keeping neonates in total darkness. We propose that dark rearing (DR) creates a ‘metabolic sump’ that depletes O2 in the retina and consequently maintains normal retinal vascularisation during O2 supplementation.The work undertaken in my PhD thesis aims to test this hypothesis, and ultimately, we intend our findings will have immediate translational outcomes for clinical management of ROP and benefit an entire population of premature infants.
