The Care Manager is responsible for the Care Coordination of Targeted Case Management-HH Cases or the Children’s Waiver-HH cases. This includes assessment of new referrals, outreach to gain referrals and coordinating medical and mental health services with assigned Health Home.

The Community Psychiatric Support & Treatment worker (CPST) provides activities that are intended to assist the child/youth and family/caregivers to achieve stability and functional improvement in daily living, personal recovery and/or resilience, family and interpersonal relationships in school and community integration. CPST can also be provided in coordination with clinical treatment services, to address identified rehabilitative needs within a comprehensive treatment plan. Services are delivered in a trauma informed, culturally and linguistically competent manner.

I specializes working with clients struggling with depression and anxiety. I utilizes motivational interviewing techniques and cognitive behavior therapy to help client's gain awareness of their triggers and gain insight on how to use their in-born strengths to decrease symptoms. Through warmth and compassion, I ensures that all clients feel safe, supported, heard, and connected to the appropriate resources.

Provide appropriate skill building activities designed to assist seriously emotionally disturbedchildren in acquiring, developing and accessing the function skills and supports needed tofunction more successfully in community environments such as the family, school andcommunity.◘ Provide appropriate respite activities designed to assist seriously emotionally disturbedchildren and their families in receiving a necessary break to ease to stress at home and providefor family harmony.◘ Contribute to the safety and therapeutic environment of the program.◘ Supervise the spending of funds during respite activities and provide receipts for money used.
