• Coordinate and help the patients, family, and other team members.• Assess the social condition of the patient and provide counseling.• Arrange financial support for patients' treatment from the government and non-governmental organizations.• Advise the patients and family, persons with disability, on current government benefits and help them to get these benefits.• Maintained liaison with the hospital team.• Help the person with a disability/illness to adjust to his environment.• Review inpatient insurance that is requested daily, utilize the acuity assessment form and note down review outcomes, contacts, and requests for further review times for follow-up.• Physician orders and signatures on transfer sheets, financial verification, insurance approval, and transfer permits will need to be obtained, and transportation arranged.• Follow-up with the patient family to stabilize the gains made during treatment involving casework, one-to-one counseling method, Group Work, and Community Organization.• Create awareness among the people to stimulate people’s participation in health care programs.• Organize Blood donation camps at regular intervals.• Educate and interact with the community to help the community adjust to patients having disabilities or illnesses.
