As a Community Health Worker (CHW I) I worked to provide ECM (Enhanced Care Management) services to low income and/or MediCal patients through Bay Area Community Health ECM and HPS (Homeless Prevention Service) programs. By monitoring, guiding, and mediating patients with their health insurance benefits, CHW's enable patients to more securely and independently self-advocate when confronting complex diseases and illness, and improving their general health and well being. Some tasks as a CHW included:-Conduct outreach and enrollment services for patients in ECM-Perform patient screening surveys, score questionnaires and alert care team- Serve as a liaison between the community and health and social services.-Maintain logs as directed by supervisors-Navigate EPIC for documentation, book appointments in Electronic Health Record, according to provider's scope of practice-Connect patients to internal resources and/or services to meet their needs

• Conducted a year-long study to assess the reliability and validity of Suitability Assessment Methods (SAM) and SMOG readability for health materials.• Implemented SAM and SMOG tools to enhance the literacy of health materials on the Women’s Mobile Health Unit, a project part of Cal Poly KPH and NOOR foundation• Collaborated with student volunteer research team to analyze and rate the effectiveness of health materials, contributing to improved patient education and engagement.

My main role as a laboratory processor is to process blood, urine, and other human specimens for BPA Research Study at the Cal Poly Center of Health Research (CHR). I would participate in this 3-4 times a week throughout the academic year.

As a student phlebotomist working in the Cal Poly's Center for Health Research (CHR), I would conduct mainly blood draws for purposes of A1C testing for various studies done within the CHR (e.g. BPA study, Healthy Mamas, etc), in addition to doing draws on the Women's Mobile Health Unit which I had previously volunteered on. I would occasionally do post-collection processing for both blood and non-blood (i.e. urine) patient samples, in addition to transporting samples to off-site labs.
