Act as liaison between the school and the community in the development of internship opportunities(paid and unpaid work experiences) for JAG students.• Serve as a resource to provide support for JAG graduates and their families as these student’stransition from high school to college and/or other post-secondary opportunities throughout the one year of follow-up (e.g. workforce, apprenticeship program, etc• Establishes ongoing partnerships with, businesses and industry, organizations or community agenciesto strengthen student learning and community engagement within the JAG schools.• Assists in the development of community service activities for JAG students to educate the communityregarding the JAG program.• Works with all school personnel and the staff at the WorkOne Centers, to coordinate employer contactsand resources.• Coordinates guest speakers and field visits for JAG class and individual JAG students.• Maintains communication with mentor organizations, employers and community organizations• Create awareness and appreciation for college within the high schools• Attends team meetings, open houses, job fairs, college fairs and recruitment events.• Pulls data for all reports that need to be compiled and submitted ensuring all deadlines are met.• Ability to compile, analyze and evaluate data, make determinations, and present findings in oral orwritten form.

• Assess safety and level of risk of a child. • Perform needs assessments to determine treatment options for families and children evaluated to beabused/neglected or referred by law enforcement, other social service agencies or schools.• Testify in either a Juvenile or Criminal Court of Law concerning the needs of the families and childrenthat are assigned to the individual’s caseload.• Develop case plans collaboratively with families to assist them in becoming more self-sufficient ina safe and nurturing environment and to ensure a permanent home for the child. • Monitor treatment plans of contractual service providers to determine if they are consistent with thecase plan.• Develop discharge/transition programming to assist families and children to attain and maintain self-sufficiency in a safe environment.• Assess the service capability of residential treatment programs and providers to ensure that they meetthe needs of children and their families.• Coordinate local service delivery for a family or children to assist them towards the establishment ofa safe and stable home for children. Facilitates team meetings with the child and family along with their identified natural and formal supports to direct case planning.• Counsel families regarding the social needs and skills of families and children.• Conduct home studies, family histories and recommends placement for adoption to an adoption teamand assumes lead role in decision making regarding the family to be selected to adopt the child.• Visit children in out-of-home placements to coordinate family reunification, adoption or independentliving efforts.
