I am working as a PCN pharmacist in two GP surgeries based in Bracknell. My role is to provide face to face medication reviews, telephone consultations, manage long term conditions, prescription queries and care home ward rounds.My responsibilities include:• Work as part of a multi-disciplinary team to provide specialist pharmaceutical support and to improve care and support of patients.• Promote safe, evidence-based, cost-effective prescribing in line with CCG requirements.• Maintain complete records of patients using appropriate clinical templates and coding.• Manage repeat prescription requests, resolving queries and ensuring patients have appropriate monitoring tests in place.• Competent in providing services like covid and flu vaccinations.

• Provide prescribed medications, drugs, and other pharmaceuticals as needed for adequate patient care.• Provision of enhanced services like emergency contraception, influenza, and travel vaccinations.• Communicate policy and procedure changes, system enhancements and clinical updates to all pharmacy staff.• Supervise the fulfilment of prescriptions by pharmacy technicians.• Supervise healthcare assistants in the sale and supply of GSL and P medicines.• Ensure adequate control and documentation of all controlled substance records and monitor patient medication profiles• Provide pharmacy services for substance misusers including supervised and unsupervised supply of Methadone.• Maintaining a legible record of CPD's according to the standards and guidance of General Pharmaceutical Council (GPhC).• Counselling and advising patients on the treatment of minor ailments.• Advising patients of any adverse side-effects of medicines or potential interactions with other medicines.
