Consults with supervisor and RN as appropriate, in order to determine a level of care decision that is in the least restrictive setting for the consumer.Works closely with other social service agencies, hospitals, clinics, nursing homes, personal care homes, assisted living facilities, and other community agencies in the assessment of the needs of the consumer.Submission of required information into the PA Department of Aging standardized computerized data entry system, SAMS and OMNIA. Effective use of computers required.Obtains records from mental health, mental retardation and other related service providers for consumers where OBRA assessments are required. Makes arrangements with the RN Supervisor for evaluation of records obtained are not sufficient to make a recommendation on the consumer’s need for active treatment.Consults with psychiatrist, psychologist and social worker, as appropriate, in order to make a recommendation on consumer’s need for active treatment if they meet the OBRA criteria.Forwards all assessment information to the appropriate regional field office for review. Interfaces with staff of regional offices as needed.Prepares and maintains case files according to Department of Aging regulations.Maintains ongoing documentation of case contacts and actions taken for each consumer in a standardized and organized manner.Participates in joint visits, case conferences and scheduled meetings as necessary.Explains agency programs to individuals, groups and agencies as requested.Prepares for and attends appeal hearings at the Department of Public Welfare.Performs related work as required.
