•Evaluate each participant and work with them to determine what types of supportive services the participant needs in order to obtain or remain in permanent housing.•Complete an Individualized Service Plan for each program participant.•Provide outreach and case management services and assist participants in obtaining VA benefits and other public benefits, which may include: health care services, personal financial planning services, transportation services, fiduciary and payee services, and legal services.•Assess participant’s need for emergency assistance and provide assistance within the framework of Hope Manor II support services.•Meet with each program participant face-to-face as needed to provide support, monitor progress on the service plan, etc.•Maintain updated progress notes and manage participant files as determined by program policies.•Deliver services with sensitivity to the participant’s cultural and socioeconomic characteristics.•Administer training curriculum when needed and assist in identifying potential employment opportunities for program participants. •Attend outreach events within the community to actively recruit eligible Veterans.•Effectively communicate and collaborate with Hope Manor’s VA Liaison and Hope Manor Property Management.•Assist in cultivating ongoing relationships with other service providers (such as mental health and substance abuse providers) and providing important referral and resource information to co-workers to ensure the most up-to-date and comprehensive database of resources for Veterans in the program.•Represent the interests of the agency, the program and the program participants in all activities by upholding VOA-IL’s mission and values.

• Meet with each assigned veteran face-to-face to provide support, work towards employment goals, and monitor progress• Provide pre-employment assistance and support, including working on resume, practicing interview skills, locating employment opportunities, and addressing specific barriers to employment and self-sufficiency• Provide career counseling and job coaching through determining specific industries and career which will be a good fit for a veteran, coordinating job skills training as outlined in each employment plan, fully preparing veteran for interviews with employers, and working with veteran on job retention issues• Provide ongoing retention support by providing mentoring and support to veterans for the first year of employment, which may include: facilitating transportation assistance, offering conflict resolution, providing encouragement and emotional support, and mentoring the veteran in sustaining positive lifestyle changes• Regularly participate in outreach efforts to actively recruit eligible veterans for VOA-IL employment program• Attend community-based activities as needed and appropriate (i.e. stand downs, job fairs, and outreach efforts to employers)• Establish and maintain complete and accurate records of client information and progress towards goals and objectives• Assist in cultivating ongoing relationships with other service providers (such as area VA medical centers, mental health and substance abuse providers)• Providing important referral and resource information to coworkers to ensure the most up-to-date and comprehensive database of resources for veterans in the program• Represent the interests of the agency, the program, and the program participants in all activities• Invest in creating and supporting a healthy and successful workplace culture• Consistently upholding the mission and values of VOA-IL
