• Diagnose, Evaluate and Treat Psychiatric and Substance Use Disorders for 13YO and older with charting (Credible) • Serving underserved population needs for Serious and Persistent Mental Illness, IDD, Gen Psych and MAT • Lead MAT Team meetings to include RN, Therapist and Admin personnel • Provide MAT developmental education to providers, nursing, therapists and staff across state-wide organization • Network with community resources to market MAT program, lead education efforts about MAT in community and create strategic business partnerships for patient care for MAT services

• General Psychiatry & Addiction Medicine medication management 18YO and older • Facility support services of TMS Therapy, Spravato Treatment, SAIOP, psychotherapy and psychiatric evaluations for medical procedures such as bariatric or hormone replacement

• Integrated Treatment for Co-Occurring Disorders Behavioral Health Clinic offering psychiatry, counseling, & addiction medicine• Addiction Medicine (Sublocade & other Buprenorphine products, Vivitrol/Naltrexone etc.)o Outpatient alcohol, opioid and other substances detoxo Buprenorphine DEA-X provider• General Psychiatry* Transitioned ownership to Eleanor Health 9/2021 and became Staff MAT provider while closing CBH
