Assist person supported with daily living requirements including but not limited to meal preparation, medication administration, and personal care. Manage significant social and behavioral challenges in a supportive, non-restrictive manner. Provide transportation and scheduling assistance for medical appointments, therapy sessions, and social outings. Document and account for all financial transactions related to person supported. Implement and document all supports as specified in the Individual Support Plan.

Conducting daily social and recreational activities with clients in accordance with the daily activity schedule and individual program plans. Assisting clients with activities of daily living including dressing, feeding, toileting. Assisting clients on and off vehicles. Assisting with meal time activities. Observing client response and reporting all pertinent signs, symptoms, changes, etc., immediately to supervisor. Participating in the planning and implementation of individual program goals. Providing first aid and emergency care as required.

Provide a supportive, caring, safe environment by attending to individual needs while respecting differences and supporting growth and independence.Offer input into the planning and development of outcomes with regard to the practical implementation of outcomes.Assist each resident with personal hygiene, dressing, eating, and all other activities of daily living.Assist in the implementation of Individualized Support Plans and related activities (social, recreational, educational, and developmental).Compliance with corporate policies and procedures and Office of Developmental Programs regulations.Attend to the residents’ emotional, physical, or medical needs.Report concerns regarding health, safety, and well-being to management.Report incidents of suspected abuse, neglect or other as required by the policy on Incident Management.Obtain 24 hours of required training per training year.Actively participate in all aspects of job development.Attend and actively participate in staff meetings.Regular attendance at work.Other duties as assigned.
