The leads work to improve care for people with chronic kidney disease by providing strategic guidance in the regional implementation of the ORP, leading quality improvement strategies and engaging primary care providers, nephrologists and other stakeholders in their regions.- Achieved a successful transition of the medical care of the patients of the Headwaters Healthcare Centre dialysis unit from Credit Valley Hospital (CVH) physicians to William Osler health system by conducting a staged transition informed by two joint rounds, one with the physician leader of CVH and the second with the RML of Mississauga-Halton, thus providing patients with ample notice and opportunity to allow questions, concerns and contingencies to be addressed. The resultant transfer of accountability proceeded without any clinical, administrative or legal disruptions.

The program in translational medicine, 'Access to Science in Hemodialysis', provided education at provincial and international levels. - Supported the creation of the first in-center nocturnal hemodialysis program addressing the needs of patients who required prolonged dialysis treatments. Relieved the scheduling and resource tensions by generating the capacity for 40 hemodialysis patients without the need for further capital or infrastructure construction. The program became the prototype since adopted by several Canadian programs.
