Manage a caseload of 35 residents at the Brookville Hotel with 250 men with housing, mental health, physical developmental disabilities, medical and behavioral needs, reporting to the Program Director. Work with NYC agencies to provide services from HPD, HUD, DHCR, HFA, DHS, DOHMH. Deliver daily case management services for residents including Psychosocial Assessments, Service Plans, Mental Health Evaluation Referrals, Counseling, Supportive Housing, Medicaid, Health Insurance, Medical Appointments, SNAP Food Stamps, Supplemental Income, Public Assistance, Employment, Recreational Activities, etc.Review medical healthcare and mental health evaluation records and request new assessments from Psychiatrists. Provide Counseling, Crisis Intervention, Safety Assessments, Career Assessments, Strength and Needs Assessments. Document and log physical and electronic records in software database system CARES for quality assurance. Conduct reports and internal audit review of documents to ensure adherence to State/Federal Regulations HIPPA. Establish and maintain ongoing relationships with public city agencies concerning public benefits, employment services, supportive services, housing resources, etc Participate in leadership team meetings which includes Executives and Directors, Social Workers, Counselors, Behavioral Coaches, Case Managers, Residential Advisors, Operations Managers, and Administrative Staff Teams. Complete mandatory trainings on Crisis Emergency Management, Emergency Housing, CPR, Fire Safety, Harassment, Abuse, Safety, and other NYS Mandated Reporter Training in compliance with NYC requirements.
