Under supervision, provides and coordinates the care provided to transplant patients in the post transplant phase of care. Serves as a fully effective and participative member of the multidisciplinary patient care services team. Demonstrates and models the full range of professional practice in the delivery of patient care and in participation of the development of strategies to improve patient outcomes. Accountable to patients and families for the processes and outcomes of care during the post-transplant phase of care. . Triages all post-transplant outpatient phone calls, guiding patient care through coordination of multidisciplinary aspects.Coordinates scheduling of all post hospitalization medical procedures, diagnostic testing, clinic visits, and medicine changes deemed necessary by physician and/or protocol.
