Implementing Coordinated Entry and Youth Housing Navigation. Working with a team to case manage adults and youth who are homeless or at risk of homelessness. Lead Agency for Coordinated Entry. Assisting in prioritizing clients based of their needs and barriers. Helping clients navigate services with our agency and with other neighboring agencies, creating a safe and positive atmosphere for folks to address their needs and barriers. Helping others create a positive place in the community, ending with the resources they need to navigate most services in our area.
