Facilitate patient discharges, navigate client conversations around treatment plans and financial obligations. Provide client education including, but not limited to: home care instructions, medications (indications, adverse reactions) and general wellness updates on hospitalized patients. Ensure patient records are accurate and complete. Assist in end of life care for patients.
