- Conduct approximately 20 pre- kidney and/or pancreas initial psychosocial transplant assessments per month to assess psychosocial appropriateness for transplant- Develop clinically-based impressions and recommend psychosocial interventions to prepare patients for transplant dependent upon the need and nature of each individual patient- Collaborate with multidisciplinary teams to develop and recommend effective interventions to prepare patients for transplant- Maintain a caseload of 1500 post transplant patients to ensure ongoing post-transplant success- Assess needs of pre- and post- patients and families and refer them to appropriate community resources, mental health services, and other resources as needed. - Implemented weekly transplant social work staffing meetings and lead weekly meetings to facilitate open communication and transparency of social work needs and schedules- Collaborated with fellow transplant social workers on re-development of support group programs for pre- and post- kidney/pancreas transplant recipients
