We currently manage four community hubs in Gateshead and our Community Linking Project operates across 4 PCN’s in the Borough providing intensive support for patients with non-clinical issues. We have recently launched our Health Equity team, with a range of opportunites to tackle inequality in our Borough. Currently I am leading across sectors (Health, Local Authority, Police and VCSE) to develop Integrated Neighbourhood Teams in each of our PCN areas.
