• Oversee Population Health, Nurse Care Management, and Clinical Pharmacy programs for a large primary care accountable care organization (ACO) serving more than 120,000 patients• Collaborate with executive leadership, directors, and staff on care delivery design and operations to drive Coastal Medical’s clinical and financial performance across multiple shared savings contracts• Devised strategies that reduced the cost of care by over $55 million for Coastal Patients in all-payer shared savings contracts in first 5 years ($28M MSSP)• Created platform to deliver strategic population health goals across 20 practice sites under value-based payment models. Lead cross-discipline teams redesigning care for high-risk and rising risk patients to achieve quadruple aim goals of better care, better health, lower cost, and increased provider satisfaction• Accountable for the strategic development, successful implementation, and on-going performance of ACO initiatives.
