SUPERVISOR: Executive DirectorHOURS/SCHEDULE: Up to 40 hours per weekDUTIES: Directly responsible for providing crisis intervention services to individuals who have been hurt or harmed including but not limited to, outreach as required, crisis counseling, coordination of emergency shelter, food, clothing, accompaniment to law enforcement or medical services as necessary, and transportation. Directly or indirectly responsible for assisting clients to negotiate the legal system including, but not limited to, assisting clients to complete and file protection and antiharassment orders, coordinating, translation or interpretation assistance as required, assisting clients in gathering information needed for court, coordination with other service providers, agencies, or programs, accompanying clients to court, explaining the legal process, and provide transportation. Directly responsible for regular ongoing counseling and case management services including, assessment, case planning and implementation, regular case management and counseling sessions, information and referral services, support, advocacy, crisis intervention, and termination planning and implementation. Directly responsible for maintenance, of complete and accurate documentation including, but not limited to, intake files client files and progress notes, services rendered, and daily, monthly, and quarterly statistics and reports. Mutually responsible for community education and awareness, community and public relations including public speaking as required. Attendance at weekend and after-hour events required.Participate in weekly staff meetings, regular supervision and on-going training.

Responsible for the coordination and provision of services to foster children, foster families, and foster adoptive families, and promote foster care services within the community.

Provides a range of clinical and/or case management services to a diverse CHSW consumer population including participation in assessment of individual consumer needs. Participates in the implementation of treatment/service plans. Duties and Responsibilities include, but are not limited to: Develops and implements goals and objectives of a CHSW Program in coordination with CHSW's strategic plan; Ensures that all records and statistics are appropriately maintained and reported to ensure compliance with governmental grant regulations; Provides for quality assurance or center-directed services, including a parent eval component; Develops program planning and implementation with other Regional Programs and Teams to facilitate integrated services and access for families; Works closely with community organizations and schools to ensure a coordinated family support program to assist the project's targeted population; Maintain a working knowledge of activities for all family participants; Oversees the day-to-day operational requirements of the program or a family center and its satellite support facilities; Becomes intimately familiar with the WISe model and provides services from a strengths based perspective; Models strengths based perspective for CHSW staff and other service providers; Ensures the WISe principles are being met each family served in the program; Coordinates and runs the Child and Family Team meetings for each WISe participant; Conducts outreach into the community to educate referral sources on the program; Advocates for the values of a diverse population within CHSW and the community; Performs other duties as assigned.

I worked alone to assess clients, refer them to all the MH housing providers. It was my job ensure their ability to maintain tenure with in the community. This was a very high-profile job with in the larger mental health system. It took I time management skills effectually, ability to work in stressful conditions and work independently without supervision . majority of my time in the field Screening for housing eligibility . This job allowed me to gain knowledge on how to work prioritize & get the job done in a timely manner. I worked closely with hospital staff, developed and maintained good report & coordinate care with a number of team members .

Facilitate group and individual activities for the chronic mentally ill and provide residence with training and activities of daily living to support their efforts towards recovery. Position assist the multidisciplinary treatment team and providing residential services to 60 residence, working with families, community agencies, DDA case managers. as provided mental health treatment assessments, daily case noting, assess residence needs and develop and implement treatment plans. Provided geriatric residents and their families with resources and help with Social Security, Medicare, paperwork required by the state as well as constant case noting.

Vacation Care Program: In its most basic sense, vacation care is an out of school care center, which offers activities for primary school aged children. The program has become so successful that an extra 10 places have been allocated for the following year, bringing the daily attendances to 40 children.
